Today we’d like to introduce you to Paulami Guha.
Hi Paulami, we’d love for you to start by introducing yourself.
I rebuilt my entire professional life in the United States from the ground up after already completing my medical education and residency training in India. Repeating years of training, competing again for residency as an international medical graduate, and then pursuing a surgical fellowship required not only intelligence and skill, but also persistence, resilience, and endurance.
Sixteen years of medical education and training across two countries is far more than a career path — it reflects sacrifice on many levels. I left behind an established trajectory in India to adapt to an entirely new healthcare system, culture, and professional environment, while continuing forward despite uncertainty, setbacks, and immense personal and professional challenges.
The fact that I ultimately matched into OBGYN — one of the more competitive and demanding specialties for foreign medical graduates — and then advanced into surgical fellowship training in Jacksonville speaks to the determination and resilience that shaped my journey.
My story reflects a version of the American dream that people rarely see from the outside. Many notice the final title or professional success, but not the years of examinations, overnight calls, visa stress, financial pressure, rebuilding professional identity, and starting over while peers elsewhere had already become established attendings.
Today, I have built a successful career as a complex gynecologic surgeon in Jacksonville and am deeply proud of the women-owned private practice my colleague, Mary Ellen Wechter, and I have built together, alongside our exceptional manager, Misty Sell. Our practice now spans multiple hospitals and office locations throughout Jacksonville and has grown into a thriving and respected presence in women’s healthcare. We have become a trusted referral destination for physicians across the Jacksonville area who entrust us with the care of patients requiring complex gynecologic surgery.
My journey is proof that reinvention is possible, even when the road becomes far longer and more difficult than expected.
I’m sure it wasn’t obstacle-free, but would you say the journey has been fairly smooth so far?
My struggles…
Starting over professionally
The challenge of being an international medical graduate (IMG)
Extensive years of training
Immigration and visa-related uncertainty
Adapting to a new healthcare system and culture
Emotional and identity-related challenges
Financial sacrifice
Physical and mental exhaustion
Building a practice from the ground up
Balancing ambition with perseverance
Thanks – so what else should our readers know about North Florida Gynecology Specialists LLC?
We are one of the few practices in the North East Florida region offering complex pelvic surgery using both robotic-assisted technology and advanced laparoscopy. These minimally invasive approaches allow for faster recovery, less postoperative pain, and a quicker return to daily life and work compared to traditional open surgical techniques.
Our practice is 100% women-led and women-run, and we are deeply committed to comprehensive care across all stages of a woman’s life. In addition to providing advanced surgical care, we support patients through adolescence, contraception counseling, reproductive health, and the transition into menopause.
We aim to provide care that is not only technically advanced but also personalized, compassionate, and continuous across every stage of life.
Who else deserves credit in your story?
I could not have done this without the support of my family — my husband and my daughter — whose encouragement and grounding presence have been essential throughout this journey.
Our successful practice is the result of the combined efforts of many dedicated individuals. It is built on the hard work and collaboration between my partner, Mary Ellen Wechter, my manager Misty Sell, and the strong support of our management companies, Femwell and Vitals MD.
Our surgical practice would not function at its level without the dedication of our surgical scheduler, Brianna Evans, and our surgical assistant, Sarah Adams. A significant part of our postoperative care success is also due to our nurse practitioner, Jessica Stickens.
I am also deeply grateful to our clinical supervisor, Liza Logan, whose guidance, assistance, and compassionate care have been invaluable in helping us maintain high standards of patient care and operational excellence.
Finally, I want to acknowledge our ever-smiling front desk and scheduling team, April Ford and Linda Jones, who play an essential role in ensuring that our patients are welcomed and supported from the very first point of contact.
