Hi, I’m Chelsea Albert, the owner of Perfect Placement Advising. I’m based in Jacksonville, Florida, and I serve families throughout Jacksonville and the surrounding areas. I live in Neptune Beach, and I truly feel honored to do this work in the same community I call home.
My passion for supporting older adults began long before my career did. I was raised by my grandmother, and as a young girl I often accompanied her to work as she cared for seniors as a private caregiver. Watching her serve people with patience, dignity, and compassion left a lasting imprint on me—and it’s still the foundation of how I approach this work today.
Professionally, I’ve worked in Occupational Therapy since 2011, with a strong focus in geriatrics across skilled nursing, home health, and transitional care. Over the years, I’ve walked alongside seniors and families during some of life’s most emotional and uncertain moments—after a fall, during a sudden decline, following a hospitalization, or when it becomes clear that more support is needed than can safely be provided at home.
I also bring additional training to my work as a trained End-of-Life Doula, a certified health coach, and someone who actively facilitates caregiver support groups in the Jacksonville area. Those experiences have strengthened the way I support families—not just with logistics, but with empathy, calm guidance, and emotional understanding.
What inspired me to start Perfect Placement Advising was seeing how overwhelming the care journey can be, especially during discharge planning. Families are often expected to make major decisions quickly while trying to understand medical recommendations, levels of care, availability, and cost—all while carrying fear, guilt, stress, and exhaustion. Many don’t fully understand the difference between assisted living, memory care, and long-term care, or how licensure and care needs impact what’s truly appropriate. And so often, families don’t realize there may be financial resources available—like Medicaid waiver programs or VA benefits—that can help make the right care more accessible.
That’s why I do what I do. I help families find the right fit with clarity and confidence, while also supporting case managers and care teams by making transitions smoother, faster, and more human.
At the heart of it, I strive to be a steady, compassionate guide for families—whether they’re facing a sudden crisis or planning thoughtfully for the future.
I’m sure it wasn’t obstacle-free, but would you say the journey has been fairly smooth so far?
I grew up around caregiving, so I’ve always felt a deep responsibility to help people. In my work with geriatrics, one of the hardest things was watching families feel scared and overwhelmed—especially during discharge planning—when they’re expected to make huge decisions fast. That emotional weight stays with you. And when I started Perfect Placement Advising, I had to take a big leap of faith and learn everything from scratch—systems, contracts, relationships, and building trust. It wasn’t easy, but it’s made me stronger, more grounded, and even more committed to supporting families with compassion and clarity.

Brand-wise, what I’m most proud of is that Perfect Placement Advising is built on trust, integrity, and relationships. I don’t believe in “quick placements.” I believe in the right placement—where someone can feel safe, respected, and supported, and where families feel peace of mind.
What I want readers to know is this: you don’t have to figure this out alone. Whether you’re in crisis or planning ahead, my goal is to be a steady, guiding light—helping families move forward with clarity, compassion, and confidence.
Do you have any advice for those just starting out?
Yes—my biggest advice for anyone just starting out is to stay consistent, even when it feels slow in the beginning. Growth in this kind of work doesn’t happen overnight. Trust is built one relationship at a time, and momentum comes from showing up repeatedly—especially when no one is watching.
Something I wish I knew earlier is that you don’t have to have everything perfect to begin. I spent time wanting my website, systems, and materials to feel “fully ready,” but what truly builds a business is service, follow-through, and relationships. Start with what you have, stay organized, and improve as you go.
I also wish I understood sooner how important it is to protect your energy and set boundaries. When you work with families in crisis, it’s easy to carry the emotional weight home with you. You can still be compassionate and deeply supportive while learning to create healthy structure—so you can sustain the work long-term.
And lastly, I would say: lead with your heart, but run your business with clarity. Be genuine, be dependable, and don’t be afraid to advocate for your value. The right people will respect what you bring to the table, and the relationships you build will become the foundation of everything.
