Today we’d like to introduce you to Sarah Adams.
Hi Sarah, thanks for sharing your story with us. To start, maybe you can tell our readers some of your backstory.
I’ve always known I wanted to be a physician, but I didn’t initially realize I would also become an entrepreneur. I trained in Family Medicine because I loved the idea of caring for the whole person — from pediatrics to adults, prevention to chronic disease, and everything in between. Early in my career, I practiced in traditional healthcare systems and quickly saw both the strengths and the limitations of that model. I loved my patients, but I didn’t love the time constraints, insurance barriers, and the feeling that true prevention and longevity medicine were often an afterthought.
Over time, I felt called to build something different — something more personal, more proactive, and more aligned with how I believe medicine should be practiced. That’s how Palm Valley Family Medicine was born. I launched it as a Direct Primary Care practice in Ponte Vedra/Jacksonville Beach with a focus on relationship-based care, longer visits, accessibility, and a strong emphasis on longevity, hormone optimization, prevention, and whole-family wellness.
What started as a small, relationship-driven practice has grown into a community-centered brand. We now serve families across Ponte Vedra, Jacksonville Beach, and Nocatee, and I’ve expanded into collaborations and leadership roles that allow me to advocate for better primary care on a broader scale. I’m passionate about educating women on perimenopause and menopause, helping patients take control of metabolic health, and creating a space where both adults and children feel truly known by their physician.
I’m also a mom, which deeply shapes how I practice. I understand firsthand the juggle of career, family, health, and community. Building this practice has never just been about medicine — it’s been about creating a life and a model of care that supports longevity, vitality, and connection.
Today, I feel incredibly grateful. What began as a vision for more personalized care has grown into a thriving practice, meaningful partnerships, and a platform to advocate for the future of primary care. And truly, I feel like I’m just getting started.
Would you say it’s been a smooth road, and if not what are some of the biggest challenges you’ve faced along the way?
It hasn’t been a straight line. Transitioning from employed physician to practice owner required a complete mindset shift. I went from focusing solely on patient care to managing contracts, compliance, staffing, marketing, and long-term strategy.
There were moments of uncertainty — especially when making big decisions about expansion, partnerships, and scaling responsibly. Entrepreneurship requires resilience. You make decisions without always having perfect data, and you learn quickly.
That said, the challenges have been incredibly refining. They’ve pushed me to be more strategic, more disciplined, and clearer about the kind of organization I want to build. Growth isn’t always comfortable, but it’s been deeply rewarding.
Thanks – so what else should our readers know about Palm Valley Family Medicine?
Palm Valley Family Medicine is a physician-led, modern primary care practice designed for patients who want more — more access, more strategy, and more longevity in their health.
I built the practice intentionally outside of the traditional insurance-driven model so that care could be relationship-based, proactive, and high-touch. As a direct primary care practice serving Ponte Vedra, Nocatee, and the surrounding community, we provide comprehensive family medicine for adults and children — but we’re known for going deeper.
Our focus is longevity medicine, hormone optimization, metabolic health, preventative cardiovascular care, weight management, and aesthetics — all integrated within primary care. We don’t separate “feeling good” from “being healthy.” We address performance, energy, strength, and aging with the same clinical rigor as we treat chronic disease.
What sets us apart is access and intentionality. Patients have direct communication with their physician, extended visit times, and care plans built around long-term vitality — not rushed, reactive medicine. Every decision in the practice is designed around excellence, experience, and outcomes.
Brand-wise, I’m most proud that we’ve created a space that feels both sophisticated and personal. Palm Valley Family Medicine reflects my belief that healthcare can be elevated without losing warmth. It is boutique in experience but grounded in evidence-based medicine.
As a founder and CEO, my vision extends beyond a single office. I’m building a model of primary care that prioritizes autonomy, innovation, and community impact. I want readers to know that healthcare doesn’t have to feel transactional. It can be strategic. It can be empowering. And it can be designed for long-term vitality.
That’s the standard we’re setting.
Let’s talk about our city – what do you love? What do you not love?
What I love most about Jacksonville is the balance. We have coastal beauty, growing neighborhoods like Nocatee and Ponte Vedra, a thriving small business community, and yet it still feels accessible and connected. There’s space to build here — whether that’s raising a family, launching a business, or creating something new in healthcare.
I especially appreciate the entrepreneurial energy in this city. Jacksonville is large enough to support innovation, but still relational enough that collaboration feels natural. As a physician and business owner, that community support has meant everything. Patients truly value local leadership, and there’s a strong culture of people showing up for one another.
What I like least — and really see as opportunity — is that we’re still growing into our identity in certain areas. Jacksonville sometimes flies under the radar compared to other Florida cities, and I think we deserve more recognition for the talent, leadership, and innovation happening here. I’d love to see continued investment in walkability, wellness infrastructure, and community gathering spaces that support connection and longevity.
Overall, it’s a city with momentum — and I’m proud to be building here.
